The misdemeanor charge against 26-year-old Danielle Weems was filed Wednesday in Cleveland Municipal Court.
An Oct. 24 arraignment hearing has been scheduled. Weems court records do not list an attorney.
Police say the boy had been left sleeping in the back seat of the van in a driveway while Weems was visiting someone at a home.
Officers say someone stole the van on Tuesday, shortly after midnight.
The unharmed boy was still inside the vehicle when it was later found about a mile away in a different home's backyard.
A judge earlier this year sentenced Weems to 30 days in jail in a child endangering case.
